Your PAE was graded by a Training Officer and was reviewed twice by the Training Division Command. I apologize, but there is nothing wrong with marking your PAE.

The question should be interpreted as if you are currently a member of any faction on any of your characters.
The second question should be answered with all your previous faction memberships on all of your characters.

Yes, since you are officially not in the LSFD. Whenever you pass recruitment and get invited in either of the two factions, double faction permission should be requested.
