• An authorization letter produced by the individual, and;
• A scanned copy of the person's driver's license.

The authorization letter must state that the person is authorizing you to request their criminal history record, along with their signature and the date when the letter was signed. The letter must be signed within the last 30 days.

We do, however, given the fact the person in question is a minor, still require you to attach the following document:

• An authorization letter from the minor's parent or legal guardian, allowing you to request the criminal record for their protégé or;
• Any other document (i.e. power of attorney) that would demonstrate your authorization to request criminal record on their behalf.
