The Los Santos Police Department thanks you for your interest and time in completing this form.

    2. Employee's name, serial number (if known) and assignment (Their rank, name of police station or location where employee is assigned.)
      • Name: Jacob Hall

    3. What initiated your contact with the employee?
      • Police response to your call

        Traffic collision
Made a report at a police station

        Visit by detective
        Witness at a police investigation
        Public Relations event

    4. What would you like to commend about the employee's performance?

I'm a Sergeant with the San Andreas State Parks, at the time contact was initiated Police Officer Two was actively rendering aid to a injured ranger that was shot in the neck, along with an EMT from the Fire Department. As I was told, Officer Jacob Hall fell out of the pursuit line at the time Lieutenant Matt Graham from State Parks, was shot and unconscious, actively bleeding out.
Officer Jacob Hall was shot in his left shoulder, however, this did not stop him in rendering the aid required to Lieutenant Matt Graham at the time. He set aside his own injuries to render aid.

It is with that that I wish to commend Officer Jacob Hall, for brave actions taken and having been involved with the life saving of Lieutenant Matt Graham, which at the time of this letter's writing, is actively recovering within Pillbox Medical Centre. The commendation is seconded by Captain Rick Graham, Lieutenant Matt Graham's brother, and was very impressed with the officer's act of duty at the time he found out what happened.

I believe had it not been for Officer Jacob Hall, Lieutenant Matt Graham would have passed away on scene due to the state of his injuries.
